Transferring Records

 If you need records released to a new physician, please sign a release of information at the new provider's location and have it faxed or emailed to us: pediatriccenterofcantonrecords@gmail.com 

Please be aware that while turnaround time is usually within a week or two, it may take up to 90 days to transfer your child's records.
